Please ensure you have picked up ALL repeats on your current script (this can be checked by contacting your pharmacy) prior to making a script request

Use this form for script requests ONLY

Not all script requests can be processed, especially for stimulant medications (in line with legislation and Health Dept regulations, including the requirement for regular medical reviews)

Please note that stimulant prescriptions expire after 6 months. It is your responsibility to ensure you have a follow up appointment in place to avoid a break in treatment.

This form can be used to request your psychiatrist to complete paperwork/documentation. Generally this is to request completion of forms for university / TAFE, department of transport, schools etc.
